St. Luke's Hospital in Columbus, NC, is seeking to add a physical therapist to our rehab department (a $250.00 referral bonus is being offered for this position).

Job Summary:
The physical therapist will be responsible for providing physical therapy services to patients/clients who have physical impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease, or other causes; provide prevention and wellness services, including screenings and health promotion.

Essential Functions:
-Physical therapists demonstrate and articulate sound clinical reasoning in synthesizing complex evaluation data identifying the diagnosis and developing a comprehensive plan of care.
-Develop and update appropriate plan of care in collaboration with the patient/family and related to the person's age and lifestyle with measurable long- and short-term goals and a defined discharge plan.
-Understand the role of physical therapy in the context of the patient's age, total needs perspective and environment.
-Perform age appropriate competency skills in area of practice and with respect to individual patient's problems.
-Actively support and participate in patient coverage efforts across all divisions within the department as necessary.
-Assess the learning needs and capabilities of patients and caregivers.
-Recognize contraindications for treatment to assure patient safety.
-Initiate patient and family education in a timely manner relative to age, LOS, readiness to learn, and emotional status.
-Utilize appropriate methods and materials for effective learning, monitoring response to education, and adapting program appropriately.
-Patient care documentation is timely, legible, and efficient.
-Documentation is concise, pertinent and meets the professions and departments documentation standards.
-Oral reporting is clear, concise, relevant, and timely.
-Assists with data collection for departmental improvement activities accurately and within expected time frame.
-Perform administrative tasks as assigned by the Director of Rehabilitation Services in order to assist in the overall operations of the department
-Establishes, monitors and delivers physical therapy to patients as directed by the physician's plan of treatment and the interdisciplinary patient plan of care.
-Reports on the condition of the patient to the appropriate supervisor and other staff members.
-Performs physical therapy assessment and procedures which require substantial specialized knowledge, judgment and skill based upon principles of psychological, biological, physical and social sciences. This includes a knowledge of growth and development, and an understanding of the range of treatment needed by patients, whether pediatric, adult, or geriatric. Responsible for making decisions based on the PT's education preparation and experience in physical therapy.
-Coordinates the care of all assigned patients to assess and identify needs and review the St. Luke's Hospital range of services resulting in achievement of expected goals.
-Actively participates in case conferences and making suggestions to team and supervisor appropriately when needed.
-Effectively communicates with all levels of the work force, both clinical and nonclinical.
Performs and prioritizes multiple functions or tasks.
-Reads and interprets technical instructions related to the care of the patient.
-Visually and auditorially observes and assesses the patient.
-Effectively deals with multiple changes.
-Builds an outpatient and inpatient PT patient base under direction of Rehab Director.
-Follow a written patient care plan and make modifications of treatment that are consistent with the plan to assure that proper treatment is given.
-Provide treatment using established techniques to assure that the patient benefit is maximized and safe.
-Recognize contraindications for treatment to assure patient safety.
-Communicate patient status to the appropriate person(s) so that those involved in the patients care and discharge plans will have necessary information.
-Write progress notes so that patient's treatment and status are documented accurately.
-Write and instruct home programs to assure that discharge plans are complete.
-Assign work to and supervise PT Tech and students to ensure that they function within the limits of their skills and knowledge.
-Utilize proper body mechanics in order to avoid injury.
-Transfer and transport patients in a manner that assures safety for both patient and self.
-Complete department records and charges in a manner that assures accuracy and timeliness.
-Contribute to the departmental improvement initiatives through participation in monitoring and evaluation activities.
-Maintain the department in an orderly manner by cleaning and storing equipment per procedure or assisting the PT Tech in these duties.
-Assist in maintaining the department in readiness for patient care by assuring adequate supplies and preparing treatment areas for the next patient promptly.
-Assists with ensuring that patient appointments are scheduled so that patient waiting time is minimal and staff work time is coordinated efficiently.
-Demonstrate knowledge of all hospital and department safety, maintenance, disaster, and infection control policies and procedures to assure a safe working environment.
-Perform administrative tasks as assigned by the Director of Rehabilitation Services in order to assist in the overall operations of the department.
-Adapts intervention environment, tools, materials, and activities according to the needs of the individual and his or her socio-cultural context.
-Communicates and interacts with other team members and the individual's family or caregivers.
-Monitors own performance and identifies supervisory needs.
-Maintains records and documentation required by work settings.
-Modifies treatment approaches to reflect changing needs.
-Formulates discontinuation and follow-up plans.
-Provide outreach and learning activities as requested which may include: in-services or related activities, generating referrals, attending community-based events, developing written community outreach materials, interacting with volunteers or visitors.
-Ensures compliance with TJC, CMS, NC State Law and APTA guidelines for practice.
-Perform other related duties incidental to the work described herein.

Required Knowledge, skills and abilities (not an inclusive list)

1. A minimum of a bachelor's degree in Physical Therapy is required.
2. Successful completion of NC Physical Therapy State Board Examination or other state equivalent is required.
3. Licensure as a Physical Therapist within the state of North Carolina is required.
4. Must hold and maintain current BCLS certification.
5. Initiative and good judgment are required when performing any duty.
6. Educated on and compliant with HIPAA regulations; maintains strict confidentiality of client information.
7. Complies with Infection Control, Standard Precautions and OSHA standards for the healthcare professional.
8. Able to wear personal protective equipment (e.g., face mask, goggles, latex/non-latex gloves) as needed.
9. Ability to work effectively within role independently and with other team members.
10. Understanding of medical/surgical terminology.
