often choose to make their wishes about end-of-life care and healthcare
decisions known. For some, this means talking to their doctor, family
members, and friends. Others complete documents called advance directives.
If you come into the hospital as a patient, make a copy of your advance
directives. Bring these with you. We will place them on your chart so
the health care team can know about the choices that you have made.
Living Will
A living will is an advance directive that informs your physician of your
wishes if you are ever terminally or incurably ill or in a persistive
vegetative state.

Health Care Power of Attorney (HCPOA)
A HCPOA allows you to appoint a decision-maker who can make healthcare
decisions for you during times when you are unable to make such decisions
